Continuing  Healthcare Advocacy

Some people with long-term complex health needs qualify for free social care arranged and funded solely by the NHS. This is known as NHS continuing healthcare (CHC) funding . You can find out more about this funding at the NHS Continuing Healthcare website.

Often the process is complex, challenging and difficult to navigate on your own, or on behalf of a family member. As people who qualify for CHC funding have long-term complex health needs this makes it even more difficult.

CHC advocacy

Our advocate can work with you to help you understand the process, and make sure that all of your needs are understood. They can come with you to relevant meetings and assessments, or if you or your relative are too ill to do this, they can go on your behalf.

There are a number of processes the NHS will undertake to check if you are eligible; including something called a Decision Support Tool, and a CHC assessment. The advocate will make sure you are clear about what these are, and how your needs fit into them.

If you're a professional working with someone who you think is eligible for CHC funding please contact us to discuss a referral, or make a referral by using the referral form.
